Every conclusion is a compression of reality. A document, a specification, a policy or a price is the residue of reasoning that is no longer visible. The first stage records what arrived, exactly as it arrived, without interpretation.

Procedure
The artefact is entered without commentary. Claims, scope, date, authorship and provenance are recorded. Nothing is corrected at this stage.
Compression is not a fault. It is the normal end state of work. Recording it precisely establishes what was actually presented, so that reconstruction can be checked against it.
Provenance is stated. Where provenance is unknown, confidence is reduced accordingly.
